Case Manager

Summary of Responsibilities

The Case Manager/Career Coach is responsible for assessing client’s needs for resources and services on an outpatient basis, coordinating supportive services, and providing career readiness training, consultation and support to ensure that Project LIFT program participants receive maximum benefit from Project LIFT’s unique combination of substance abuse and mental health treatment and vocational training.

Specific Duties & Responsibilities

The specific duties will include:

  • Performing needs assessments to determine each client’s level of physical and mental functioning, needs for support and services, and areas to target for improvement.
  • Conduct and complete needs assessment for every participant within 2 weeks of start date or at discretion of clinical team; ensuring that needs are identified and met prior to discharge. Ex. resource to therapy, mock interview, etc.
  • Evaluate and prioritize needs of participants connecting resources to their needs (food, shelter, clothing, water top priority).
  • Assisting with coordination with community resources, maintaining relationships with community service providers, including implementation of monthly roundtable discussions with community resource partners and proactively reaching out to resources in the area with quarterly touchpoints with each contact.
  • Providing guidance and assisting Career Risers and After School participants with work readiness initiatives regarding job readiness skills, job searching, creation of resumes, interviewing techniques and follow up. Conduct ONET Assessment and track in Apricot.
  • Assisting with development of Workforce Readiness Curriculum.
  • Delivery of the Workforce Readiness Curriculum to Career Riser weekly following the Instructor Guide, including “Word of the Week” initiative.
  • Developing and delivering group counseling modules to assist program participants in adopting skills needed for success in the workplace, including time management, effective communication, anger management and assertiveness, etc.
  • Assist with college applications/scholarship applications, assist with tutoring resources; track in Apricot, as needed.
  • Participant Follow-Up (Post Employment) – Follow-up with participant & input into Apricot weekly for one month and then monthly for 3 months, then quarterly for one year.
  • Weekly Touchpoints– record in Apricot a goal of 50% of weekly participants in attendance; including participating in programming and capturing additional touchpoints.
  • Provide feedback to therapists and program coordinators on behaviors, barriers, situations of participants.
  • Record participant case management notes into Apricot within 24 hours to include needs assessment.
  • Provide safe and reliable transportation for participants to get to appointments related to case management to include Driver’s License bureau, social security office, etc.
  • Attend meetings on time, providing participant updates to appropriate staff and presenting any barriers for services.
  • Attend Juvenile Court at the discretion of Site Director.
  • Assisting with program evaluation framework to assess the strengths of the program and identify areas of improvement.
  • Perform other duties as determined by the Director of Clinical Services, including projects and assisting on occasion with meal preparation, providing transportation for enrolled clients, facilitating relationships with mentors, etc..

  • Demonstrates ability to develop and maintain rapport with clients and family members.
  • Maintains high level of confidentiality and professionalism.
  • Maintains current knowledge of substance abuse and mental health treatment as well as applicable laws, regulations and standards for juvenile case management.
  • Detail oriented with excellent organizational and time management skills.
  • Ability to determine work priorities and ensure proper completion of work assignments.
  • Effective verbal and written communication, teamwork and presentation skills.
  • Ability to establish and maintain effective working relationships with the general public, co-workers and members of diverse cultural and linguistic backgrounds regardless of race, religion, age, gender or disability.

Qualifications

Bachelor’s degree or higher in mental health, social work, criminal justice, or related field. Associate’s degree with 5+ years of relevant experience and/or with CAC or CAP may be considered.

Physical Activity/Working Conditions

While performing the duties of this job, the employee is regularly required to sit, talk and hear. The employee is frequently required to use hands to finger, handle, or feel and reach with hands and arms. The employee is also required to sit at a desk and use a computer. The employee is occasionally required to stand and walk. Specific vision abilities required by this job include close vision and ability to adjust focus. The noise level in the work environment varies. In addition, the employee must be able to lift up to a minimum of 25lbs, able to work in non-airconditioned environment and to stay on feet for extended periods of time.
